By providing your phone number to Unity Health, you expressly consent to receive phone calls from us and our licensed insurance partners regarding Medicare plans and services. This includes:

Note: This consent is separate from text message consent and can be revoked by calling (555) 123-4567.

Text Message Consent

By opting in to receive text messages, you specifically consent to receive SMS messages from Unity Health about Medicare information. This consent is separate from phone call consent and includes:

  • Medicare enrollment deadline reminders
  • Plan benefit updates and changes
  • Educational content about Medicare options
  • Appointment reminders

Message and data rates may apply. You can opt out at any time by replying STOP to any message.

Email Consent

By providing your email address, you consent to receive email communications from Unity Health. This consent is separate from phone and text message consent and includes:

  • Medicare plan information and comparisons
  • Educational newsletters about Medicare
  • Important Medicare deadline notifications
  • Follow-up information after consultations

You can unsubscribe from emails at any time by clicking the unsubscribe link in any email.
